Need inpt treatment : It sounds like you have developed a severe dependence and tolerance to the med. I would try to get into an inpatient treatment facility as soon as possible to begin slow detox & withdrawal from it. It would also be good to have meds reevaluated. You should be on mood stabilizer tx for the bipolar disorder. Fluvoxamine can help for 
ocd but that med and Cymbalta (duloxetine) can make bipolar d/o worse.

The salt in this meal can cause some degree of dehydration, making your heart beat faster. Also, any anxiety/panicky feelings, along with your cardiac history may have contributed to the spike. Since this was followed by indigestion, there may have been degree of pain that your body was feeling at the time. Take your blood pressure on a day when you are feeling calm. See your doctor if your blood pressure is consistently higher than your normal blood pressure.

i have strong side effects (bad eye sight, sleepiness) while taking 10 mg of fluoxetine (3rd week now). i take it against depression and panic attacks. is it possible to reduce to 10 mg every second day in the evening?

Anxiety shows itself: In may ways.  Once can mentally obsess over things that frighten the person or do behaviors to keep danger away, e.g., ritual behaviors, checking and re-checking.  There can be heart palpitations, shortness of breath, sweating, sleeplessness, GI symptoms, etc.  Please see doctor and then go to a mental health professional to learn ways to deal w/anxiety. No: People having panic attacks typically feel like they're going to die, but they don't.  Anxiety and panic produce many physical symptoms, and people with anxiety are usually extremely sensitive to body sensations . . . .Usually interpreting them as indicators of dire illness.  Panic attacks can definitely be treated.  If not already working with a psychiatrist on yours, please consider it.

Not necessarily : They can resolve with proper treatment. Medication plus cognitive behavioral therapy to identify and modify the cause of the panic attacks.

Unlikely: Levemir (insulin detemir) a long acting basal Insulin does not have know side effects of inducing panel attacks directly.  Panic attacks can be 2ndary from overwhelming emotional response that's been triggered.
