Many causes: Palpitations a unpleasant sensationof irregular and/or forceful beating of the heart.It can be due to several reason
caffeine, tobacco, alcohol use
illicit drugs like coccaine, amphetamines, pcp and marijuana
stress and anxiety
some prescription meds
overactive thyroid gland
, anemia
changes in hormone level in women before or after menopause
gall stones, gerd
cardiac arrythmias 
other heart diseases.

Sometimes: Palpitations or the awareness of your heart beat is very common.  It may be due to extra beats of the heart from stress, or med/diet issues.  Others include atrial fibrillation which may often appear as you age.  And, paroxysmal atrial tachycardia which may be inherited or be related to heart wiring issues at birth.  Sudden cardiac death my occur in some families due to prolonged qt syndrome.

No : No unless cause is found like medication herbals supplements caffeine which is stopping or changed can relieve palpitations

Hard to say.: Physiologically there is no real affect of a single abnormal beat of the heart.  Psychologically everyone is different.  Some patients live in constant fear of another abnormal beat ( which they may feel could lead to some horrific death) while others are completely unaware that they have 30 abnormal beats every minute.  Truly amazing variation in awareness.

In general...: Palpitations are the feeling in your chest of an irregular heart beat.  Most of the time they are benign.  If bothersome or very frequent, your doctor can get an EKG, echocardiogram and lab work (thyroid and electrolytes).  Oftentimes a monitor is necessary to see what your heart is doing when you feel these.

Many things...: Anything from anxiety, to abnormal motility problem of the esophagus..Tube leading from mouth to stomach lying right next to heart, which is another cause--a heart/rhythm problem. Get it checked by a trusted, respected pcp.  (for those following my answers, it should become obvious that the latter should be key to  a good state of health).
