Yes: The vaginal area becomes less lubricated with age and starts to atrophy . Speculum examination may cause pain and result in mild bleeding. If bleeding persist or worsens see your doctor.

Specimen collection: I trust you mean pap smear test. If the specimen for pap is collected properly a urinary infection should not affect the results, however, it would be better to treat UTI first and then do the pap test.

Yes: Past exposure and infection with wart producing strains can be confirmed or excluded by blood tests that look specifically for those strains. The "smear" by itself is a simple way to look for cervical changes due to other HPV strains associated with cervical cancer (not wart strains).

Confusion re problem: you are confused with regard to causes of not having a period.  Primary amenorrhea describes post-pubertal women without their first period. Women whose periods have stopped have secondary amenorrhea.  In young women (yours dates to age 16) the cause may be intense physical conditioning, or other causes, but not "abnormal cells in the uterus.  Please obtain a gynecological examination.

Infertility/amenorrh: Ultrasound can detect the reason behind irregular periods sometimes. The workup is stepwise beginning with history and physical. The medical provider will check blood work for hormone levels.  You've had the pregnancy test and pap smear done so your pcp will probably order an ultrasound soon. If everything is normal then male causes of infertility will be explored. That's the usual pattern.

Not related: Pap smears are done to prevent cervical cancer. The idea is to detect abnormalities long before cancer occurs. Cervical cancer is caused by the sexually transmitted HPV virus so it is not something based on family history. It is also not related to PCOS. Typically the first pap is done after age 21. Your obgyn can help you with PCOS and give you options for cycle regulation

Depends: The speed of your pap smear results depends on the laboratory which is analyzing the sample.  You should, however, usually have the results in no more than a week.  If you haven't heard, call your provider.
