Simple possibility : Without evaluation I would guess a boney prominence on top of the foot is rubbing the shoe.  If the case try skipping an eyelet when lacing the shoe.

Random Pain: if there has been no injury, then you may be dealing with a superficial nerve problem. In addition, you should have an xray done to see if there is a boney prominence that may be injury your tendons on the top of your foot.

Positional.: Where is the cramping and severe pain located when the foot id elevated. Elevation should relieve most problems. The fact that pain and cramping increase with elevation could indicate a problem with under perfusion of tissues with blood. To remove all doubt, have you foot evaluated to consider all possibilities.

Probably Tendonitis: Even though your mris were negative, low grade tendonitis of the extensor tendons on the top of the feet  can be negative on mris. If the top of your foot hurts to pull your toes up and/or down, probably 95% chance of tendonitis.

Stop Playing around.: Sounds like there's a whole lot going on here. Any one of the areas of pain could be the problem and compensation causing the rest. 
Could be soft tissue or bone or both. Your best bet is to have it examined to eliminate the possibilities. My guess is that you will need a cam walker of stabilize the foot for a while.

Compensation: The pain in the foot and ankle may be due to compensation. Meaning you may be walking differently due to the knee pain causing other lower extemity joints to hurt.

A thorough history and physical exam and possibly some laboratory and radiology exams are needed to determine the exact cause. Causes of pain may include: trauma (fracture, sprain, arthritis), infection, metabolic conditions (gout), musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy).

Pain: Plantar fascia or heel bruise common. A trial of a heel cup or arch support would be a good first step. If persistent visit with podiatrist best idea.
