Lumbar radiculopathy: Pain and tingling sensation down one leg with associated same sided low back pain is most often cause by a pinched nerve. Nerves pass through the lower part of your spinal column before entering and providing nervous supply to your legs. The nerve can get pinched or compressed, usually by an out of place disc,  before they enter the leg. This can in turn cause the symptoms your are experiencing.

See your doctor.: This is one of those problems where a visit to your doctor is necessary to figure out what's going on.  Only after a thorough evaluation, including examination and possibly labs and other tests, can your doctor correctly diagnose you and treat you effectively.

Depends on multiple : Factors. It appears your symptoms are mainly leg pain, and hence a surgeon may have decided that the l45 was the only level with stenosis or pressure on nerve, and wanted to to do only what was needed at the time, and not what maybe needed in the future. Difficult to tell without seeing your prior images, but could be an okay decision. Now, you have to consult with a well trained spine surgeon.

i had a laminectomy three weeks ago to remedy nerve pain in my left leg.  after surgery the pain was still there and continues.  what should i now exp?

S/p surgery pain: F/U with  surgeon who did the laminectomy  or second opinion. . May need imaging/ CT to assess status of  healing and if there any collections  of fluid.  If bowel of bladder issues go to ER  immediately.  May  need additional injections/ treatment. On occasion surgery does not completely resolve the situation.

foraminal stenosis at l5-s1 but emg is normal. massive pain on left leg, between buttocks and foot. why emg shows normal?

Radiculitis: Good question. An EMG shows if there is essentially nerve damage to the extent that the nerve is starting to no longer connect to the various muscle fibers being checked with the EMG needle.  If instead the nerve is just "irritated" by a bulging disc or bony facet spur than the nerve will still be healthy enough to maintain the connection to the muscle and the EMG result will be normal.

Hip?: Just based on the location it might be your hip. Is the pain related to activities? Did it follow intense exercise- running, biking, etc?. Would suggest relative rest and further monitoring of your symptoms.
