Could be ......: The organs on either side of diaphragm can give sensation of chest heaviness that radiates into back. Gallbladder, fatty liver, gastric distention, ulcers, pneumonia, pleural fluid, and more. Get evaluated with your regular doc, take a friend with you, ask for a Ultrasound of abdomen and CXR. Go sooner if fever or vomiting, be well.

They : They can be. Disk herniations can be felt as back pain, or more typically, pain radiating to the buttocks or lower extremities. You may feel numbness, tingling, pain, weakness, buzzing, or electrical shocks down into your legs. These are highly suggestive of a disk herniation. Back pain itself is not very specific for any one particular problem. If you notice that you have radiating pain in your legs or numbness that worsens when you sit or lift an object, or when you bend over, these might all point to a disk herniation. It sounds like your pain is focused in your back, and unfortunately that alone is not really enough to tell what is causing the pain.

Neck: It is true that muscle strain is the most common cause of acute pain in the neck after injury.  However other sources of pain include the discs, joints and nerves.  If the pain doesn't go away you should be reevaluated.

Evaluation: Best advice is to get a physical exam with a spine specialist.  Make sure that the neck gets a complete evaluation as well, since many neck conditions can have referred pain to the upper back and scapula regions.

Tension: Number one cause for pain radiating into neck from back is stress and tension in neck and upper back muscles. Differential is much more than can be answered here. Reduce caffeine esp energy drinks and develope stress relief exercise for neck and upper back. Drink more water and look into getting a better nights sleep.

Should have it checked: If it is right under the skin or slightly deeper you should have it checked by a physician.  It may get infected or grow larger in the future and become a bigger problem

Neck pain: Have you been in any kind of an accident? Do you play sports? Hopefully your pain is just caused by muscle spasms but you need to see the Dr to make sure there is nothing serious going on in your spine. You are too young for this type of thing. Feel Better soon

Two likely reasons : A 63 year old can have shoulder, neck, and back pain for many reasons. Most common is degenerative disease (arthritis) of the spine, shoulder, rotator cuff. Weak, deconditioned muscles can also hurt with fatigue. Both are often treated with physical therapy exercises, anti inflammatory meds. Any pain that persists or cannot be relieved should be evaluated with an exam and imaging (mri, xray, etc).
