Nerve: Several considerations, such as nerve problem in neck shoulder region, or even distally in arm.  See your doctor to find out if this might be cervical radiculopathy, thoracic outlet syndrome, carpal tunnel syndrome, or notalgia paresthetica.  If this left sided, check for cardiac issues.

An Ortho evaluation: When patients have persistent pain a good physical and exam can usually help figure out the causes which in this situation can include rotator cuff issues, neck pain, a pinched nerve or muscle spasm. An orthopedist or physical medicine and rehab can evaluate if a MRI is necessary and when to start therapy or other treatment. There are other non ortho causes of pain to consider like costochondritis.

Have anexam: The lymph nodes in your arm pits may be fighting some sort of infection and your depleted condition and tension/fatigue could have affected your back. See a Dr to find the cause(s) and get some relief. While I'd be silly to declare that you don't have cancer latent somewhere - I don't see any reason to think you do. Ask the doctor who does your exam/lab work to evaluate that possibility. Best!

Pain: Be careful of treating x Rays only. I need a good physical exam to determine possible pain generators and conservative treatment  probably also need thoracic xray with pain around shoulder blades

Sorting this out: You may have issues in neck and worthwhile getting neck films.  Perhaps see your doctor and consider physical therapy.  Use muscle relaxant and local warmth.  Hope this is a valuable approach.

See below: At your age pain in shoulder blades is usually due to muscle strain/fatige from poor posture or in some cases it's related to your neck or mid back.  In rare cases it could be referred pain from your internal organs.  See a physician if your pain lasts greater than 3 weeks.

Muscle spasm: Spasm of muscles in one are often causes other muscles to tense up. Hot compresses may help and you may take over the counter pain medication.
