: Since you have so many possible pain generators, would explore with your doctor, whether you have hip labral tear, whether testicle pain due to kidney stone, whether hernia has recurred, whether low back has worsened.  Good luck.

Warm soaks: Likely an infected hair follicle or sebaceous cyst. F it heads and drains with soaks it doesn't need antibiotics, but if if progresses it needs to be seen by an md.

Testicle torsion: This may represent testicle torsion or testicle congestion. Epididymitis also could present this way as can orchitis, a testicle infection.

Testis pain = see Dr: Testicle pain means a trip to the doctor right away because the testicle's blood supply can be twisted off (called torsion). When that happens, the testicle can die. Other causes of testicle pain include trauma, infection, twisting of an appendix of a testicle (a small lump next to a testicle), etc... Since one usually cannot tell if his testicle is twisted, an exam by the doctor is needed.

Need to be examined: Any testicular swelling  need to be examined by a physician , lower is not the problem could be normal , swelling of it is the issue ,  to make sure is not a testicular mass see your doctor.

Back and or hernia: The lifting could have caused an inguinal hernia or you might have a lumbar spine issue which can cause pain to radiate into the testicle
