See a doctor: Pain during a bowel movement and with a full bladder may signal an infection. There are other possibilities, and you will need to see a health care provider to sort this out.

Probably not: If pain during normal bowel movements is your only symptom it is unlikely the be irritable bowel syndrome. Consider rectal fistula or rectal abscess if the pain occurs when stool is exiting. If the pain occurs when straining and it is on the left side consider descending colon pathology. If this persists, at least a colonoscopy should be done, and possible a ct of the abdomen.

Anal concerns...: The various causes of anal pain is broad, however common causes include benign conditions such as hemorrhoids or anal fissure.  Hemorrhoids are swollen anal veins that may or may not be visible externally.  A fissure characteristically causes sharp pain and is often associated with rectal bleeding.  Another common cause of anal pain is called protalgia fugax caused by pelvic floor muscle spasms.

during a bowel movement, when i push, i notice a fair amount of pain in the left side of my penis (not really in the testes). what could this be?

Referred pain: This may be hard stool inside the sigmoid colon pressing through the wall on a portion of the left pudendal nerve; a branch supplies the area you describe. If you have other left lower quadrant pain / tenderness, get with your physician for possible diverticulitis. I'd be interested in hearing from any gastroenterologists / urologists on here. Thanks.

Muscle strain: sounds like a muscle strain.  Doesn't have to be a hernia (feeling for bulge).  If fever, change in bowel habits or vomiting related to it, than likely internal and not a muscle issue.

i have 1 small external hemorrhoid on the right side of anus from 2 months. it only swells during bowel movement and i feel it otherwise it is very small. it causes me itchy and sometimes a sharp pain instantly for few seconds. how to treat it?

: Anusol often helps relieve those symptoms. If you continue to have discomfort see your GI who may be able to offer a procedure that might eliminate the problem

Could?: Almost anything 'could' happen.  The question is how you know the pain is rectal and not perirectal or radiating from some other area.  You should go back and allow the surgeon who did the procedure reevaluate you and find a satisfactory answer to your problem.  Hope you feel better soon.  Must be very disturbing.

Unusual: but the innervation patterns in this area are complex. If this is the first time it has happened, I would use a heating pad or sit in warm water and see if it doesn't go away, while starting a bulky fiber diet and taking a stool softener so you won't have to strain at all.  If it keeps up, you may need to have an exam to be sure you didn't "split" the inner skin a little.  You can take Tylenol.

Increased peristalsi: Although, the information you provided is not sufficient to arrive at correct diagnosis, one explanation is that you may have increased peristalsis (contractions) of your colon (namely, cecum, portion of the colon located in the right lower abdomen in response to bowel movement. It may have to do with the consistency of your stools, how much straining you have etc.
