You should see a Dr in person who can do a hernia exam and potentially some imaging. Do not delay in being seen as entrapped bowel can lose bloodflow and die. If you cannot be seen by your Primary care Dr today, go to urgent care or the ED.

Unpleasant feeling: Testicles belong down in the scrotum. Sounds like hyper-retractile testes. You need to try & push/milk it down if & when it gets stuck up in groin above scrotum. A urologist can certainly take care of this problem with relatively minor surgery, if need be.

Testicular volume is: the measured length x width x thickness in mms of the testicle or its comparison with a standard model of the testicle.  It varies primarily by age and the way it's measured.  If the information you provided is accurate, it is NOT normal for a 34 y/o male..  The lower limit of "normal" is 20ml, the average 30ml &amp; the upper limit is  40ml. The man in question needs to speak with his doctor.

Fever or no fever: No fever: could be intermittent testis torsion, torsion of appendix testis (onset for either would be sudden), spermatocele (you might be able to feel a small spherical structure above the testis) or varicocele (feels like a "bag of worms when standing and goes away when you are lying down) with fever: epididymitis or epididymo-orchitis (pain onset not sudden).
