Pain: Two months of dull pain without progression of symptoms could be muscle in origin. It is best to have it checked by your doctor to be on safe side.

YES!: An out of place rib can definitely cause pain like this. Go online and buy a MA Roller. About $30?
It's a specially shaped roller that you can lay on a carpeted floor on the roller and slowly adjust your body to move the roller up and/or down your back. It will adjust your spine and ribs using your bodies weight. It's wooden and can be quite painful over the area that's a problem but it will help.

Get seen now!: Pain that is that severe means that you have to get seen STAT. So go to the ER now, please!  Pain in the back and shoulder could be coming from an abdominal cause.  Even heart disease can present this way, especially in women.  So get seen now.  You don't want to get worse, do you?

See your family MD: Could be a herniated disk in the neck causing your pain or in your upper thoracic spine.
Could also be muscle spasm with referred pain.

Multiple causes: Upper back & left sided chest may be the result of a thoracic disc herniation, thoracic facet syndrome, T4 syndrome, costoveterbral syndrome, muscle strain, ect. It may also represent referred pain from internal organs such as the heart & lungs, which are more serious. With your breathing difficulties you should contact  your  pcp for an immediate evaluation. If not available go to nearest er!
