Possibly : Yes, it is likely a sign of muscle strain/spasm. But, there are also other potential causes. If you've done recent heavy lifting/strenuous exercise, had a cold wherein you coughed a lot, etc, then this is very much possible. Heart / Neck problem: Heart problems should be checked for first.  Though, if it was a heart problem, you would likely been to the ER/MD already.
Neck problems can cause a pinched nerve and pain that radiates to shoulder blade and down the arm quite commonly.

Dr evaluation : Pain that persists &amp; wakes you from sleep should be evaluated.  If you have had an injury that could be the cause, but if not it could be a nerve irritation possible from shingles (rash should come out about now tho) or something else that might require xray imaging to discover . Heart / neck problem: Should see your doctor ASAP to make sure your heart is OK as your symptoms are very typical for a heart attack although a heart attack is extremely rare in a 27 year old female.
Neck problems causing a pinched nerve sometimes may cause similar symptoms.

Pain neck, back: The type of radiating pain you describe is often from nerve impingement (pinching) in the neck spinal area or in the thoracic area.  See your doctor for diagnosis.  Physical therapy helps.  Avoid neck and back strain, heavy lifting most sports for now.

Chest pain: Causes for chest pain would include irritation to your left pectoralis muscle, irritaion to costal nerve or rib cage, zoster rash, or problem with costo-vertebral junction in your thorax with radiation to the front of chest. Could be referred pain from inside of body either from your heart, lungs or even esophagus.
