Not fungal: Heat rash is not a fungal infection and is best treated with cool soothing oatmeal baths and applying corn starch powder before exercising or going out in the heat. You could if needed apply Chlohexidine  2% to 4% to the affected area for 15 seconds and rinse, avoiding eye contact

I am not sure: I know the pharmacists is telling you the truth because the knowledge with topical treatment is not as with knowing that taking fluconazole increases chances of spontaneous abortion if taken during pregnancy. It does not mean you could not use something topical and be fine. Boric acid, vinegar and benzoyl peroxides are options. I gather boric acid is not safe in pregnancy.

Yes: If you have Insulin resistance or even diabetes metformin is fine to take.  Some people have lost weight on metformin, perhaps due to GI side effects.  I haven't heard that it can interfere with diet pills but those can be dangerous on their own.

None: There is no good over-the-counter medication for vaginal herpes.  All anti-viral medications are over the counter.

Not as well.: There are some over-the-counter remedies, but no guarantee that they will work as well.You can try rinsing with a baking soda solution, salt water,cider vinegar or hydrogen peroxide. However these are not good options if you are treating a baby. Gentian violet is also another option, but is a bit messy. As with any meds, please discuss with your doctor.

No: The only thing that works for oas is allergy shots to reduce your sensitivity. I don't know exactly why that is, but i've never met a single patient who had found any medication that prevented oas symptoms.

Hemorrhoids: There are no oral treatments for hemorrhoids. Try to keep bowels soft with plenty of fluid and high fiber. 
Creams are over the counter as well as available as suppositories.

Scalp ringworm: You can try tinactin (tolnaftate) or Nizoral cream. Ring worm in this area of the body can be hard to get rid of and you may need to get a prescription from you doctor if those are not working.
