Palliative/ hospice : I wish I had better news, but your mom' s condition is serious. Depending on her functional and nutritional status/ cancer stage she might be a candidate for hospice care if congruent with her wishes. At a minimum she should have a Palliative Care consult in order to make sure her symptoms are well controlled and her further care is aligned with her and your wishes.

Need more testing: The fluid should be drained ( if big enough) and sent for laboratory and pathologic analysis to determine its nature. You will also need restating ct scans and recheck tumor markers ( ca 125).

May be due to spread: Plural cavity ( the space around lungs) normally will have very small amount fluid for lubrication around the lungs and chest wall.  When detectable amount accumulates could be due to cancer cells deposits or could be due to heart&lung diseases( non cancers).

Any cancer in lung: Any cancer that originates in the lung or spreads to lung from other organs can cause pleural effusion. Other than primary lung cancer, cancers from intestine, liver, adrenal, prostate, pancreas, stomach etc can all spread to the lungs.

Several causes: Pleural effusions can be caused by infections, congestive heart failure and sometimes cancer. Asbestos exposure and autoimmune diseases are other causes. Sampling the fluid and having it analyzed is recommended.

Absolutely: Two reasons to drain it are for diagnosis, are there cancer cells in the effusion, this is bad. Bumps you up to stage 4. Two, therapeutically it will be easier to breathe if the effusion is removed.

Drain fluid: Pleural effusion is fluid that collects in the cavity that holds the lungs.  When fluid builds up, it can prevent the lung from expanding fully so you can breathe.  Worst case scenario is the lung can collapse.  A special tube can be placed in the chest to drain this fluid.  You should be working closely with your oncologist and a thoracic surgeon.

: Suggest further imaging with CT/MRI under your doc's guidance
