Probably not OHL: I think you've been reading too much information on the internet, without real understanding. Several things that can cause various marks on the tongue in addition to hairy leukoplakia. Without HIV, OHL is a minor condition. Even with HIV, it's definitely not a "death sign". HIV is rare in 19 year olds in the US. even w/ at "hi risk. See doctor or dentist, and expect a reassuring response.

Leukoplakia: Leukoplakia is white patches in the mouth on the cheeks or tongue. It is often caused by irritation from rough fillings or ill-fitting dentures. It is also caused by chewing tobacco and snuff.
Removeal of the irritation will usually cause the patches to disappear. However, they should be biopsied because they may be the start of oral cancer and need to be treated as such.

Go see a head and neck surgeon who may use medical or surgical treatments to control/remove it as it is a precancerous condition. You also need to remove the cause ?Tobacco chewing or smoking, typically.

Can't scrape it off: Leukoplakia cannot be scraped off although scraping at it could change its appearance somewhat and perhaps lead to that illusion. Oral thrush can be scraped off and some forms of hairy tongue can be significantly altered by scraping.

We hope so: White film on tongue is white film on tongue, not likely to be leukoplakia. You should be able to scrape or brush this off with floss, tongue scraper or brush (stick tongue out, place whatever on tongue near back and then pull tongue back in mouth). Leukoplakia will not wipe/scrape off and looks more like white adherent skin - think what a callous looks like after swimming. See Dentist if needed.

Very real: This would not be rare at all. You have racked up 8 years times the amount per day (measured in packs = ppd) .So, at one ppd you have accumulated 8 pack years. Multiply that by 365 days times 20 cigarettes per pack and get the total of 58,400 smokes.Think heat plus chemicals times how long it takes to smoke one cig and that is how much exposure your mouth has had. : As for your other question, see a doctor to confirm the diagnosis. Probably you have something other than hairy leukoplakia, and your tonsils may not be abnormal.
