Normal Testicular Size is as follows
Length   2inches 5cm
Width     0.8 inch 2cm
Height  1.2inch 3cm
Some are bigger than this and other may be smaller

Hydrocele: Is the swelling completely gone and is the testicle back to normal? If so the most likely thing is a hydrocele which is just a fluid collection around the testicle. The most common cause is trauma or an infection of the testicle and it is completely benign. If it gets big and small and big and small this is a communicating hydrocele or a hernia and would likely need surgery to fix.

Testicle Anatomy: It sound pretty normal to me.  One testicle is the dominant one and one is the backup in case of injury to the dominant one.  Healthy testicles are almond shaped.

No: If this has indeed happened, you should be examined right away. All adult makes should be performing routine monthly self testicular exams.

See a urologist: After an infection such as epididymo-orchitis the testicle may be swollen for some time, or a reactive hydrocele may develop around the testicle.  A urologist should evaluate you to make sure another ultrasound or further tests are not required at this time.

Sometimes yes: The connection of varicocele and male sub-fertility is not entirely consistent: though some men with sub-fertility are helped by varicocele repair, a majority of men with varicocele never have  fertility problems. Expert groups of urologists and res often disagree on specifics, so for a less general answer i recommend getting a in-person evaluation with an expert you trust.

Get eval: Assuming you mean the testicle has just gotten bigger and is now painful too (referring to your testicle as a "her"), then you need to get an evaluation asap as it could be several things, such as a varicocoele to a more serious condition that needs treating asap.  Best wishes.
