Carotid blockage.: It's not the presence or absence of the medication that matters. It's the efficacy of the medication that matters. How well are BP, blood sugar, cholesterol and weight controlled? Are we smoking cigarettes? Blockage will progress if risk factors for progression are not adequately controlled.

Unlikely.: Dissections usually recanalize withing 3 months. Sounds like the blood flow is routed to vertebrals now.

The risk of carotid blockage is when it is severely narrowed ie >80%; then a piece of plaque can break off, lodge in the brain and caues a stroke. If the artery is 100% blocked nothing can break off. The other arteries should be checked.

Symptoms: If you have symptoms of a TIA ("ministroke") or 70% or more narrowing on the left. Meanwhile, the person you're describing should be on aspirin or plavix (clopidogrel) and a statin drug (to lower cholesterol) with optimal blood pressure.

95% Blocked: Ultrasound is a great way to determine how blocked the carotid arteries are. It turns out that the more blocked they are the greater the risk for a stroke. Even if you are not having a symptoms (mini-stroke or actual stroke), a blockage of 80% or greater is enough to warrant a carotid endarterectomy. If you are having symptoms, then a blockage of 50% or greater is enough to warrant surgery.

Probably good: Statin drugs such as Crestor (rosuvastatin) have a beneficial effect upon arteriosclerosis; current research is focusing upon the brain effects since arteriosclerosis is a systemic disease.

Medical management: For most partial occlusions of the carotid arteries of <50%, aggressive medical management is usually all that is needed (i.e. No surgery.) this will include aggressive cholesterol control with LDL < 70 mg/dl and HDL > 45 mg/dl.  In addition, blood pressure control with systolic < 130 mmhg and diastolic < 80 mmhg (<130/80). These are general recommendations; see your doctor for your specific case.
