Stroke: It is the same as stroke . Caused by impaired oxygenation to the brain or part of it with sequential loss of function of that part and possible death sometimes. Caused by either loss of blood flow by clot in main arteries or emboli which are clots or particles concluding main or distal branches or from bleeding into the brain which is the worst outcome.

Needs to be seen..: Since I don't know all the details and other symptoms associated with this numbness neither I know the time frame this has been going on for I would suggest that if this is recent go to the ED for immediate evaluation. This could be related to a pinged nerve, stroke, brain lesion, infection, among others.

Incidental finding: The white matter changes are not related to your head concussion, they are rather chronic and have to be interpreted in the clinical context. Talk to your doc at follow up about the results and try to recall any unexplained symptoms (if any), in trying to determine if they are of clinical significance.

Foreign Body?: Although anything is possible, it is unlikely that a simple laceration sustained several years ago would still cause discomfort unless contaminated with a piece of glass or debris, or if the laceration involved a nerve which hasn't healed properly.
