Needs oral med: Ring worm in the scalp which causes loss of hai in that area more common in children but can happen in adults, needs to be treated with oral antifungal drug like grsiofulvin or Lamisil (terbinafine) and as the infection clears completely the hair will grow back.
Niozoral shampoo is also used along with oral med which will prevent the growth of fungi as well as kill the pathogen.But shampoo alone will not work.

Doubt it: Generally the fungus is entrenched in the hair root and eradication will require meds that are delivered through the blood.The topical agents can suppress the fungus but are unlikely to clear it. I'd consult another doc.

It's probably not: ringworm.  Nizoral (ketoconazole) pills are dangerous to the liver so I would get in to derm, you may just have alopecia areata

Yes: Follow OTC directions on box or a doctors prescriptions for best outcome. You can always do your own thing with OTC pharmacy meds. They are safe to use. The pharmacist can guide you if you get stuck.

See below: Ringworm on the scalp would present with swelling in the area (often boggy), areas of broken hairs at the base looking like black dots and redness.  If you have this, you need treatment for the ringworm.

DHT: Dihydrotestoterone, is a hormone, excess levels of which have been implicated in male hair loss and prostate problems. Ketoconazole may produce a decrease in testosterone making it useful for treating prostate cancer and there are studies suggesting efficacy in regrowing hair.
