Nitroglycerin: Nitroglycerin relieves  cardiac related chest pain by dilating the coronary arteries. These are the arteries that supply blood to the heart. Improving blood flow to heart muscle will relieve the discomfort caused by a partial blockage of the blood vessel with plaque.

Chest pain: If you have chest pain and have taken Nitro without any improvement then you should be seen by a physician right away.  You should go to an ER for evaluation.  If it is not serious then you can follow with your regular physician, if it is then you will be admitted for further testing, but don't wait.

Lying down: It is surprising to me that a 23 year old would have significant enough coronary artery disease to require these medications. Nitroglycerin will briefly lower your blood pressure more which could be detrimental. If you are lying down when you take it you may not pass out or feel as dizzy. I think you need to confirm the need for these medications with your health care provider.

Bedrest: The normal order is bed rest - light walking around the hospital room or hallway may be allowed but varies on a case to case basis.

No: It will not show up in blood work and it won't show up in your BP either because it wears off in about 10 minutes.

Ischemia: It indicates  ischemia. You may need angioplasty or coronary arthery bipass. Your cardiologist is the best guide, standards are different in different countries. Good luck.
