Depends upon size: A small nasal septum perforation may heal up on its own. It probably will not heal up if it is large. The best things you can do are to not smoke or inhale any toxic substances and stay in general good health. Exercise, healthy diet and a multivitamin are good ideas.

Septal Perforation: It is possible that your existing perforation may worsen. Try to avoid nasal decongestant sprays that may further compromise vascular supply to the septum and worsen the perforation.

Yes: Nasal septal perforations are a problem. They crust and bleed easily. Unfortunately using a nasal spray can sometimes aggravate the situation even though you angle the spray away from the perforation. You might be better to try to get the perforation fixed.  If the perforation is small fixing it is relatively easy. Seek an experienced  ENT physician who will be able to advise you in more detail.

NOSE: Can be septal perforation, allergy or other causes. Nasal injury leading to septal hematoma is one of the common causes of perforation. You need to be seen by an ENT specialist for exam and further advise.

Difficult: A septal perforation will usually cause chronic dry, crusty nose and sometimes bloody discharge.  Nasal saline, neti pot rinses can help, but sometimes nasal ointment like Bacitracin or neosporin can provide additional relief. Otherwise, see an ENT to evaluate for repair of the defect.
