There could be a problem with the nerves controlling muscles in the tongue, throat, esophagus, etc... There could be other disorders (such a swelling, looseness, tumors, etc...) in those parts of the body.

Frequently patients require much higher amount for generalized seizures control (sometimes up to 3000 mg daily divided in 1500 mg bid) and sometimes more than 1 agent is needed for a complete control.

Numerous causes: Brief list: normal jerks on falling asleep, but problem if it awakens (plms).  Associated with epilepsy (juvenile myoclonic), spinal cord inflammation (aid's), following hypoxia (lance-adams), organ failure (kidney, liver), metabolic and medication reactions.
Meds that seem to work best here include depakote, keppra, (levetiracetam) and maybe lamictal.  Other drugs used in epilepsy, may make worse.
