What problems cause: it.  
All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that,  SGPT of 74 is slightly elevated. It is an indication of damage to liver cells. If you drink alcohol, stop entirely. If you are overweight, lose weight. If you are taking any medications, discuss those with your doctor.

Yes: Hi. Yes, weight gain can cause elevated SGPT (aka ALT) in the neighborhood of 75. It is not uncommon for type 2 diabetics to have elevated SGPT due to fatty liver. However, many things can cause an elevated SGPT. You might want to talk with your doctor about your elevated SGPT and run tests as appropriate. High SGPT due to weight gain seldom causes severe liver problems, but other causes may.

Retest: Rare folks run high SGPT with normal SGOT without evident illness. Elevated transaminases without obvious cause may be due to hepatitis b/c, hemochromatosis, wilson's, autoimmune hepatitis, antitrypsin deficiency, medication effect (are you sure you know what your "anti-oxidant" really is?), others.

Benign but check US: The most common cause is a relatively benign fatty liver caused fy fatty acids and triglycerides. Check and try to lose weight.\Minimize alcohol, and check for

Yes: Need more of the other transaminases to advise. However isolated or more elevated alt may indicate liver injury your doctor may check you for hepatitis and review your medications.

???: It evades me thinking what medicine is used to lower sgpt. If the cause of the rise of liver function is known then we can target the cause but there is no medicine that targets the sgpt. Talk to the doctor to find out the diagnosis and find out how that problem is being treated. The lowering of SGPT may not be a necessary solution and may have to stay a bit elevated.

Not unusual: It's not unusual to have normal liver enzyme levels when you have cirrhosis.  These enzymes are elevated when there's is ongoing liver damage.  Cirrhosis is scarring of the liver.  If the damage to the liver has already been done, the levels may be normal.

No: In general, smoking is not the cause of elevated liver enzymes. In your medical history I see you have a diagnosis of fatty liver. That is the likely cause of elevated liver enzymes.  Other things to consider include infectious hepatitis, alcohol, toxins, tumors, and bile duct abnormalities.

If fermented!: Alcohol intake is a common cause of liver cell injury resulting in elevated sgpt. Apple juice does not damage liver cells, unless it is fermented first to turn the sugars onto alcohol.
