Probably OK: With no change over a few years, almost certainly this is not a melanoma, which undoubtedly is your fear. But if you remain concerned, you'll need to see a doctor (preferably a dermatologist) in person.

Skin malignancy ?: Irregular borders, color variation and non healing lesions all signs of potential melanoma or other skin carcinoma. Node is worrisome as well, if not painful unlikely it is related to infection so seek care with dermatologist or your primary. What you are interpreting as a lymph node could be inclusion cyst so need evaluation.

Yes: If the mole has not change in size, shape, color, has not developed a nodule or started bleeding, it is likely to be benign. Unrelated to your question - If you do not wish to be pregnant, use contraception all the time, every time. You may consider implanted contraceptive, or IUD. 
Practice safe sex. 
Get HPV vaccine.

Changes: Yes.
Changes in size, shape, irregular borders, coloration can all signifiy malignant change. See a dermatologist asap if you have mole changes.

Pigmented Nevus: Yes it may still be benign. The abcd criteria is more of a screening instrument to identify nevi worrisome for dysplasia or malignant melanoma and is not very specific. A biopsy is your surest way to tell.

Can be: Any mole in question should be looked at by a doctor.  And you should also do self exams to evaluate changes

Yes: this could be skin cancer which if seen early is very treatable.  You should see a dermatologist immediately

Skin biopsy: A mole can be partially or completely removed.  The skin is numbed and the lesion is cut out. It is then sent to pathology to look for cancerous cells.  The pathology determines whether further treatment is needed.

Yes: Based on your question, it seems like you have already had a biopsy of the lesion.  If that is the case, you have some waiting to do.  I know that this can be scary and frustrating. Take deep breaths. There is nothing to do right now.  

Once you get the results, it is important to have your doctor explain, in detail, what the results mean and what the next steps are, if any are needed.

Must be part of proc: See the DR. Do you have stitches? Bruise? Was a cautery (MONSEL sol'n) used? Betadine?  Not serious.
