Instability: First you need to figure out if it's your kneecap or your shinbone(tibia) shifting on your thigh bone (femur). Patellar instability can be initially treated with pt and a brace. Knees with a torn anterior cruciate ligament will need surgery to stabilize.

Diagnosis needed: Your symptoms sound like they are from the patello femoral joint.   The first thing you need is an accurate diagnosis from a sports medicine doctor who has expertise in this area.  Surgery is rarely necessary for most patello femoral problems and if it is recommended i would get at least one second opinion before going that route.  Usually this problem can be helped with  treatment but not cured.

: Unless your experiencing an overuse syndrome &amp; since both knees are symptomatic, I suspect an autoimmune disorder and recommend consulting a rheumatologist for evaluation, testing and treatment. However if you've been engaged in very strenuous activities, bilateral tendonitis or ligament injury are also possibilities.

Patellofemoral pain: Given your age and gender I suspect pain may be coming from around your kneecap or patellofemoral joint. This is more common in females and has a clear assoc w/ hip/quadriceps/core weakness, hamstring tightness, sometimes abnormal alignment. Mainstay of treatment is physical therapy/home exercises to correct abnormal mechanics and optimize strength.  See knee specialist to coordinate care.

Knee instability: There are many causes for knee popping.  Sometimes it is the knee cap or patella that is sliding in and out of its groove causing the popping.  Other times it can be the entire joint opening up or shifting abnormally such as a meniscus or ligament tear.  A doctor's physical exam should help provide the answer, or an MRI may be necessary.  If it is the knee cap, a strength program can fix it.
