Elevate and get exam: Several potential causes - most benign, but depends on additional history.  I recommend rolling up a cool small towel and using it to elevate your scrotum/testicles while lying down; can also take ibuprofen / naproxen. You should seek medical attention if testicles feel abnormal, you have penile discharge, burning with urination, or pain worsens/persists.  Take care.

Uncertain: From the description you are unwell. Have you used the bathroom for frequent urination or defecation?  What symptoms are you referring to by "felt sick"? Fever?  Any urinary tract symptoms? Any discharge?

: As you apparently know, STDs like gonorrhea and chlamydia cause epididymitis. But that's almost always severe pain and tenderness of only one testicle. Pain of both testicles that comes and goes probably is not epididymitis or other infection; not surprising that doxycycline didn't help. If it continues, see a urologist if not already doing so. In the meantime, this doesn't sound very worrisome.

Testis pain: Testicular pain, also known as orchalgia, can be the result of many things.  An infection is certainly a possibility.  Injury is another.  Testicular pain is very uncomfortable and should be evaluated by a urologist for treatment.

Testis pain = see Dr: Testicle pain means a trip to the doctor right away because the testicle's blood supply can be twisted off (called torsion). When that happens, the testicle can die. Other causes of testicle pain include trauma, infection, twisting of an appendix of a testicle (a small lump next to a testicle), etc... Since one usually cannot tell if his testicle is twisted, an exam by the doctor is needed.
