You got it: An infection. The anus was not designed for sex. There are numerous bacteria in there that are not "safe" for the genitalia. That's why it's called "high risk behavior" . You are at elevated risk for several std's as well and damaging your anus.

Not recommended: Anal sex can disrupt your anal sphincter and is generally not recommended by gynecologists. Is there a reason why you would not use your vagina?  Intercourse in pregnancy is usually OK as long as you do not have a problem with Pre-term labor, or placenta problems.

Not Recommended: If you have internal hemorrhoids, i would recommend against anal intercourse as this can exacerbate the issue whether lubricated or not.  If they are in fact internal hemorrhoids, you can consult a surgeon for banding to improve or resolve the issue.  Meanwhile, to reduce flares, also avoid straining and keep stools soft.

See another: First, use OTC medication as if you have a hemorrhoid. What you have is most likely a fissure, which is a tear. You need to be examined by a colorectal surgeon to determine the extent of the damages and how deep the tear is. So go and see different doctor. Meanwhile sit in a warm water in bath tub. That will relieve the pain. And make sure to keep you stool soft.

Ask your surgeon: The tear may be related to trauma, constipation, and scarring. Best to talk with your doctors who treated your anal tear. They know your case best and will give you best advise on decreasing your risk for recurrence.
