Atopic Dermatitis?: Your description is very suggestive of Atopic Dermatitis or Eczema. This condition tends to worsen with changes in weather, humidity, use of strong detergents or harsh body soap. Sometimes it may get super infected with bacteria or fungus. If moisturizers don't improve rapidly the condition, I suggest to see a doctor.

i get red itchy rashes randomly on the inside of my elbows or knees about once a month. it usually goes away in about 10 mins. should i worry? does this indicate a larger problem? i do not have any known allergies

Unlikely: If the rash goes away by itself in 10 mins, I can't think of a serious illness causing this phenomenon.

Itchy Rash: Red, Raised, Itchy bumps sounds like a description of hives. Usually oral steroids improve a hive-like rash. This type of rash can be brought on by recent viral infections, food ingestion or drug ingestion. There are, however, many causes of rash in a 3 year old child so seeing the rash would be helpful to assist in diagnosis. I would be happy to consult with your nephews mom as I am in Ohio.

Dry skin?: First try applying a moisturizer all over your body within 3 minutes after bathing and see if this helps. For the rash, topical hydrocortisone for a few days may be helpful.  However if this is a persistent or worsening problem, I would recommend a dermatology consult.

Try moisturizer: The distribution is suggestive of atopic dermatitis.   However, it is unusual not to have asthma , hay fever or both when one has atopic dermatitis.  I note that you have already tried HC cream but the OTC product may not be potent enough.  Try applying moisturizer within minutes after bathing to see if this helps.  If not consult a derm or allergist.

Need Evaluation: This can be a lot of things, first thing that comes to mind is pemphigus, a skin condition that blisters and if rubbed the skin can slough off.  You could also have a skin infection or several other problems.  You need a good dermatology evaluation.
