Be careful: There is very little data to suggest that omega-3 fish oil helps with speech delay. There was one study published in 2009 that has not be replicated. There are possible side effects to taking too much fish oil for kids, including lowering immunity and blood coagulation problems. If you really want to try fish oil, speak with your child's health provider to best understand pros and cons.

Expected not normal: During the first year, while children don't start speaking but they increasingly become vocal from learning in a nurturing environment from visual and auditory cues. You have the concern at the right age and with proper training starting now the child can expect normal speech by 3 yrs, unless there are other concerns, which may require a detailed evaluation by a developmental specialist.

Depending on his: other development and his socialization skills, he could have a more profound developmental delay. We can not make diagnoses on this platform, but I would recommend that he be evaluated for hearing loss and autism, by his pediatrician, or if available, a developmental pediatric specialist.

PANDAS : Pandas stands for pediatric autoimmune neuropsychiatric disorders associated with streptococcal infection.  This uncommon syndrome is characterized by a sudden onset of symptoms after strep infection such as 
- motor or vocal tics
- obsessions and/or compulsions
- moodiness or irritability
- sometimes separation anxiety

it usually is not associated with speech delay.  Speech delay is much more common and can be caused by many things such as a hearing disorder or expressive speech delay.  Your relative should be evaluated by his doctor for speech delay so he can receive the services and care he needs depending on the cause.  Research does show that the sooner delays are diagnosed and dealt with, the better the kids do in the long run.  Good luck.

Sounds normal: If she qualifies as high risk as a former premi or other category there may be a early childhood program that will screen her for free. After age 3, the public schools operate Child Find programs to identify such kids. Only half of what she says should be understood by strangers at 2. What you describe is not worrisome, but there are resources out there.

Autism spectrum: I would suggest an ongoing evaluation with your Pediatrician, including AAP questionnaires for your son and his teacher. Speech therapy may be helpful, and a hearing test is suggested. Limit screen time to less than one hour per day. 
If his diarrhea is severe, he needs to see a GI specialist.

Doubtful: Dont feed into the 'fits' but do work with speech or occupational therapists on other ways for him to express himself. May need a helmet if looks like he is hurting himself, which is possible but unlikely to worsen cp.
