FM: If you mean ESR =45 this may be "normal" for you.  Low  vit d is linked to many things recently but not to fm.

Stress induced: White cell count elevation can happen without inflammation, for example stress can cause elevated WBC count. So it could be a harmless white count elevation.If you are oral steroids for asthma that can cause WBC ctelevation. Make sure that your blood sugar is under good control.

How do you feel?: A low titer anti nuclear antibody level or slightly elevated sedimentation rate or C reactive protein may require follow up in today's medico legal climate but almost never indicate disease if you feel well. Go get seen.

Not usually: Blod tests for inflammation are very non specific.  The easiest way to diagnose bursitis is to image the affected joint with an MRI - non contrast.

Possibly : Those little white spots that you mention are common as we age. Without having seen them it is difficult to say but can be many different things. But certainly lupus celebrities is always at least a thought. If all  your tests point to lupus then this would as we say consistent with that. If not then what these white spots may be should widen. Yes, when localized: Mri can detect signs of inflammation, but the exam is used to evaluate a specific region of the body.  For example, if laboratory data suggest inflammation of the pancreas, an MRI exam tailored to look specifically at the pancreas can reveal signs of active inflammation, such as edema or swelling of the gland. However, MRI is not practical as a screen for someone with generalized inflammation.

Generally not good: Elevated ESR indicates inflammation and that is almost always and indication of some disease, usually an infection.  It can be elevated in normal pregnancy.

Yes .....: When we suspect inflammation we common check two tests: ESR (erythrocyte sedimentation rate) or CRP (C-reactive protein). They are quite sensitive and are often elevated in infection or systemic inflammation. But the problem is they are not specific: they don't tell you where the inflammation is or what condition is causing it. In reality, they have limited utility. Stay well:)
