mri end aug showed gallstones in gallbladder neck, now recent mri shows distended gallbladder. 13 yr son is also on tpn, emesis daily...meaning ?

Distended GB: is normal if he is not eating anything by mouth. The emesis could be from multiple things, since I am assuming there are other health issues given he is on TPN daily? However, GB trouble can cause emesis, although usually the main symptoms are right upper quadrant abd pain after meals, nausea, bloating, indigestion. He needs further workup. Good luck!

Talk to your MD: Your blood profile is unlikely related to what you're feeling unless there are other causes like hypothyroidism which can cause microcytosis. Your abdominal pain is likely related to a symptomatic gallstone, causing pain as the gallbladder contracts.Pancreatitis can develop but not currently.

No: But could give trouble, besides gall bladder attach, small stones could move into bile ducts, could cause pancreatitis , in some may not bother good luck.

Difficult: Weight loss, exercise and some meds may help to disolve stones but it can take months and the meds are expensive.  Further, the stones tend to recur when you stop the meds.  See nih or mayo clinic website on gallstones.
 Still, it is important to remember that it is not necessary to remove all gallstones.  If you watch your diet and it doesn't hurt you it can be followed without surgery.

Temporary RX : Dissolution rx can be effective for small, non-calcified stones. However, the medication (urso) is poorly tolerated. Moreover, the gallstones will return within a few years after stopping the medication. Therefore, surgical removal of the "gallstone factory" remains the gold standard.

Gallstone: Don't worry about the gallstone unless you're having problems. Typically these are intermittent, crampy right upper abdominal pain about 30-60 minutes after eating. Sometimes its at the bottom of the breast bone, sometimes goes to the back. For the rest, ask your doctor about it. Fatty liver is very common, and not progressive. Size sounds ok, but not sure which dimension. Hope this helps!

You don't: Surgery is the only viable treatment for gallstones. However, if you don't have symptoms from your gallstone you may never need surgery.

Endoscopy: This is unusual but not unheard of. The easiest way to fix this is have a doctor skilled in endoscopy open the duct and remove the stone.

Yes, but...: Why bother, when the stones may recur after dissolution? If you are dead-set against gallbladder removal, or have medical contraindications to the surgery, then stone dissolution therapy is an option. Can use oral cheno or urso) which is costly and works about 1/2 the time over 6 months (poor candidates have calcified stones), or direct infusion of mtbe into the biliary tree. 150 mm=6 inch stone?!
