Nodules: If the thyroid nodules are smaller than 10mm, than nothing other than followup may be needed.  Otherwise further workup may be needed.  Swollen lymph nodes should improve with time and followup examation or ultrasound would be warranted.  If they dont improve, further workup can be done.

Very common: Very common. It could be caused by many things- infection is the most common cause. Inflammation, autoimmune disorder, malignancy could also give you lymphadenopathy.

They Can Be!: Whenever the thyroid is under an autoimmune attack, the possibility of developing a multinodular goiter is significantly increased. Whether the goiter and nodules are from Hashimoto's or not, they need to be addressed and appropriately treated.

Enough thyroid us's have been done to develop pictures of how big each part of the gland should be at a given age in a given condition.

It depends: Upon a variety of factors, such as clinical changes, palpation by physician, prior ultrasound and lab findings, biopsy findings (if biopsy was done), personal and family history etc. Check w/your physician who should be able to tell you. Brand name T4 replacement is more reliable than your generic.

Unrelated: I don't know what would be causing a soft lump to appear intermittently on your clavicle.  Maybe it is some skin inflammation or a reactive lymph node.  It should have nothing to do with having a simple multinodular goiter.

Depends: The decision to biopsy is made between you and your treating physician. It depends on your level of comfort, what it looks like on ultrasound and whether or not it had changed from previous ultrasounds. Your choice right now is to undergo biopsy or do another ultrasound in 1 year and biopsy only if changes are seen. Both choices are reasonable.
