Individualized: Hi. No one on this site can tell you that information. You need to talk with your doctor or see a specialist who deals with diabetes. The amount of Novolog you take for meals depends meal-by-meal on what your sugar is coming into the meal, what &amp; how much you're going to eat, and your current sensitivity to insulin. You need to be doing frequent glucose monitoring to keep it safe &amp; effective.

Sliding scale insul: This is a complicated question and we cannot prescribe with this type of consult.  I would check with your regular Dr if you can't get to Endo. They know you better than we do.  You can also go to uptodate.com and review what they recommend based on all of you health issues and other meds.

I : I agree with you that it sounds like you are doing a very good job overall.  I am glad that you are so active and can do your own yard work, and feel well.

I am concerned that you are so anemic.  A hematocrit of 30.2 is very low.  It needs to be determined if that has been worked up, and if that is a stable hematocrit for you.

Your a1c sounds like it has been improving but htat is stll quite high.

I agree with you that portion control can be very helpful, and it sounds like you are quite active so that will also help you.

Humulin: Humulin is the marketed name by Eli Lilly and Company. Both are biosynthetic human insulin.
Humulin N, is neutral protamine hagedorn (NPH), an intermediate-acting insulin, has an onset of 1-3 hours, peak of 8 hours and duration of 12 to 16 hours. 
Humulin R is Regular insulin, a short-acting insulin, has an onset of 30 to 60 minutes, a peak of 2-4 hours, and a duration of 5-8 hours.

Lantis : Lantis is a slow release Insulin injection that is administered once a day, good for 24 hours. Humulin is not the same. Humulin lasts for about 14 hours and peaks differently than lantus (insulin glargine). If you were to switch to humulin, you may need to give your self 2 injections/day to regulate your blood sugar. Let your doctor know what your wishes are and that you'd like to opt for a substitute, one that is more affordable. The change may, however, change the way you administer Insulin to yourself in terms of how many injections you need in a day, as well as the frequency for which you check your blood sugar.

Not much!: Humulin n and novolin n are both forms of nph insulin.  Nph Insulin is considered intermediate-acting (generally less than 24 hours in duration, usually 8-12 hours).  They are both heavily crystallized solutions of human insulin, which is what gives it the longer-acting property (and cloudy appearance) .  Humulin is made by eli lilly & co. (usa), and novolin is made by novo nordisk (denmark).

Novalin long acting: Novalog short acting- i order them separately. The novalog covers your meals.

Yes...: Yes, novolin R is a short acting insulin while novolin N is an intermediate acting insulin - so they can be used together. While these can be purchased over the counter in most states you should still be seeing a provider to discuss insulin needs. You should not start any insulin without the recommendation by a doctor who knows your history. Insulin overdoses can kill!
