Unlikely: Heroin and its metabolites are detectable in urine for 5-7 days after last use.

Yes, both can: Don't try to treat yourself or experiment with these substances. Prescription medication (opioids included) addiction is the fastest growing addiction today. It is killing thousands. Get help, please.

A combination of: Medical and social support.  The physical withdrawal and cravings can be very intense, and medical detoxification is strongly recommended. This needs to be together with psychosocial treatment - counseling, psychological and emotional support from peers and professionals. A blocking agent (naltrexone, vivitrol) is available, and for many people, a period of maintenance with buprenorphine/methadone.

MRI scans: To see a tumor or AVM with MRI depends on many factors, including the size, shape, location, enhancement and secondary signs such as edema and changes in normal anatomy. A very small tumor in the right location can be easily seen, but  even larger tumors cannot be seen until they change the anatomy of the rest of the brain or enhance. And very small tumors at times are not seen. Same for AVM.

im 28yrs old.bin on methadone fer jus over 2 yrs im sick of it i want off it i try to miss days to see how long i can go.till i need real bad but the doc will kick me off it if i miss 3 days is there anything i can take insted of methadone everyday i don'

Methadone: Never try to do anything unless the doc who prescribes the Methadone is in the picture. I suggest you have a good talk with the doc and tell him you want to get off the methadone. He will develop a plan for this to happen.

how qualified are nurse practitioners? i see an fnp when i go into docs for illness etc. i'm always nervous they will miss something or get it wrong.

Good question: Very politically charged question.  Certainly depends on who you ask. Fnps have formalized training and certification testing.  If you feel uncomfortable in a patient/provider situation you should consider finding someone else. We still have the freedom to chose for now.

was diagnosed with d3 levels = 4 in april last year year, doc prescribed monthly uprise d3 60k, which i missed for last 4 consecutive mnths, now what?

Supplement D3.: That level is very low.  You need to supplement it somehow.  I tend to recommend the daily supplements (1000-2000 IU daily) so you don't forget.

A rheumatologist.: Your second opinion can be a rheumatologist. He or she can gather a history from you, perform a physical exam, and review your lab results in context. This way, he or she can tell you what condition you may have.   It's difficult to guess from the information you've given.
