Possible: Any infection can spread.  However, I don't see how you got diagnosed with otitis interna.  In fact, I've never heard of it.  I've heard of otitis MEDIA, but it rarely causes otitis externa.  I've never seen that, anyway.  The two can occur concurrently, but they are treated differently.

No test is perfect: Probably nobody was negligent. The chlamydia lab tests are not perfect; both false positive and false negative results can occur. What is the evidence the repeat test and not the initial one was accurate? Sorry about your husband's overreaction; even if you or he had chlamydia, it may not mean recent outside sex partners. Consider seeing a GUM specialist, e.g. at your nearest NHS GUM clinic.
