A cardiac surgeon is a type who specializes in the heart while a general thoracic surgeon does everything but the heart.  Most do both.  Historically thoracic surgeons first trained as general surgeons, but this is changing.

Armpits: Not sure, need more info such as what is the planned surgery or what is the diagnosis.  I'm a breast surgeon and I do the extra breast tissue in the armpit area, I do not do hidradenitis however, i refer to general surgery or plastic surgeon.

Not much: Usually both terms are used interchangeably but on occasions some chest surgeons do heart an sometimes the ydon't. That is the cardio component.

Entering the heart: Thoracic surgery includes procedures inside the chest, like lung, diaphragm, esophagus, ribs. Cardio thoracic procedures refer to the pericardium and the heart itself. To operate on the heart, your surgeon needs to enter your thorax/chest and then enter the pericardium and heart. Surgical work inside the heart also requires a heart lung machine in most cases.
