GPA: First of all I wish your wife all the best with full remission of the disease and uneventful maintenance of the remission. The drugs she is on are just fine so long she is followed up regularly by her rheumatologist / primary doctor for adjustment and fine tuning. There are rather more toxic drugs than what she is currently on, good that she is  responding to the current drug regimen, best wishes

Both are OK: The best medication for M cTD is the medication which treats the most active component at the time.  Both of the medications you asked about are safe to use with MCTD if they are monitored by an experienced rheumatologist.  They will both require regular lab testing and doctor visits.  You should make this choice with your personnel doctor based on your specific symptoms

NO proven benefit: You are free to consult any practitioner you believe may be able to help your situation, however there is no proven benefit to naturopathic approach to vasculitis. Often holistic approach just slower than pharmacologic, so if not in a hurry, can opt for that. Also, controls not the same on holistic herbs/meds, can have heavy metals / wrong agents / etc.

Mechanisim of med: Methotrexate pills are broken down in the liver and effect several liver enzymes.  You can get the specific details in Kelly's textbook of Rheumatology or a pharmacology text.  This effect is multiplied by alcohol which should be avoided when you take methotrexate.

See details: You can switch to the subcutaneous form of Methotrexate as this may be less likely to cause liver toxicity. You can try Arava (leflunomide) although there is also a risk of toxicity with this med. Then there is a combination of Plaquenil (hydroxychloroquine) and sulfasalazine.
