No...: If a patient is given methacholine and it results in bronchospasm, the patient is then given a bronchodilator which reverses the effect of the methacholine.

There's a list of...: Things that may lead to a false negative:  inhaled and oral bronchodilators, cromolyn, nedocromil, hydroxyzine, cetirizine, leukotriene modifiers, coffee, tea, cola drinks, and chocolate. Sometimes oral or inhaled corticosteroids are included in the list. Whether or not these things "cause" a negative depends on many factors. Methacholine test: At the national jewish  hospital in denver we did lot of research work for the diagnosis of asthma. The test can take up to about an hour. The patient has to be monitored closely during the test.
Bronchial challenge tests can also be done with histamine, mannitol and individual allergens like pollen, dust, molds etc.

It's hard: An experienced physician, pulmonologist, allergist, etc. Is able to tell when patients are not putting enough effort into a test.

Methacholin: Challenge is used if the diagnosis of asthma is in question. There is nothing to do to prepare to pass the test.you need to follow your doctors instructions exactly and stop any medications as directed.  This is a potentially dangerous test and should only be done in a pulmonary function lab or in a doctors office with those trained to treat acute bronchospasm.  It is better in ruling out asthma.

Methacholine test: The test result is available as soon as the test is completed. It can take half to one hour. A physician has to interpret the test results.
Some times the answer may not be clear cut.

Maybe not asthma: A 19 yo with a negative methacholine challenge. A methacholine challenge test is ordered when a diagnosis of asthma is suspected but cannot be proven by history or in-office simple spirometry.  A negative test usually means there is not a diagnosis of asthma. Please see your physician to determine exactly what is going on.

Medications: Yes it can by reducing inflammation and treating your asthma, bronchial responsiveness may be effected. 
Please consult with your PFT lab technician regarding which medications will need to be stopped prior to testing.

For unaltered result: Since both caffeine (which can relax the bronchial muscle tone) and smoking - (which can irritate bronchial muscle tone making it more sensitive) - can affect the test - these factors and others are to be avoided ahead of a methacholine test which is trying to guage the patient's baseline bronchial reactivity (often to investigate the presence of asthma).
