Hormonal imbalance: And not ovulating. There is help but you have to see the doctor for prescriptions. The type of help depends on if you are trying to conceive or not. Easiest is to take oral contraceptives to regulate period.

Fertility: This is not normal and it needs to be evaluated by a fertility or gynecology specialist. One of you might have a genetic (chromosome) problem. Your wife could have an abnormally shaped uterus or she may have other medical conditions which cause repeated early miscarriages. You both need to be checked.

Spotting: Irregular spotting is one of the most common problems obgyns treat. If this is one time problem it usually resolves on its own. If this is a recurring problem you should be evaluated. Infections ;  hormonal changes can cause spotting. If there is a concern for pregnancy then do  a home pregnancy test .  There are many options available to help regulate the menstrual cycle. Schedule a visit.

Could be PCOS or ...: To diagnose pcos, we need 2 out of 3 of: 1. Irregular/no periods 2. Clinical signs like acne, hair growth and/or blood tests showing too much male-type hormones  3. Ultrasound appearance of polycystic ovaries: high volumes or antral follicle counts. We check for other causes of irregular/no cycles including low egg supply, thyroid, prolactin, adrenal gland issues. See an OB or a rei specialist.

Yes : Menstrual cycles work by days, not months. The "cycle" is preparation for implantation, ovulation, removal of unused uterine lining that was ready for implantation. The timing is by days 11-14 days after the previous menses ovulation occurs.  If no implantation happens the period starts about 12-14 days later.  So your period could start on the last day of a month and continue onto the next month.

Unknown: The normal menstrual cycle does have some variance to it normally. However, see a ob-gyn md to be sure that your cycle fits the definition of "normal".

Normal variation: The variance in the cycle lengths are a function of whether that cycle produced an egg or not. Delays in  ovulation tends to  lengthen a menstrual cycle. The use of hormonal contraceptive methods will end that variance. If your cycles become longer than 35 days consistently, and you are attempting pregnancy, then see a  MD for assistance in ovulation. Post first period cycles are often irregular.
