Hypercapnea in COPD: This is the hallmark of copd.Patient can inhale , but cannot exhale completely, so when the next breath comes in the patient still has some air remaining in the lung with high content of co2. The new breath of air with o2 will not get all in the lung due to that. That why we train  patients to prolong  exhalation time to be able to evacuate the previous batch of air with high co2.

Mgt.: It would be helpful to know the actual percent predicted values on the  PFTs report, but from your description you did improve with the bronchodilator. Your physician has prescribed a long acting bronchodilator and anti inflammatory agent. The CT finding of nodules, may be related to a number of conditions, but it would be less usual for asthma. A chest radiograph and follow up may be indicated.

Still controversial: Small airway is considered the primary defect in asthma but one cannot diagnose the presence of asthma by small airway obstruction alone.  In fact it is often present in seemingly well controlled asthma. I presume that your fvc and fev-1 were normal and thus would not lose sleep over it.  COPD is also not diagnosed by this measure.
