: We can not tell a Lymphoma in small lymph nodes unless the lymph nodes are growing in size. Unless the lymph node size is bigger than 1.5-2cm, I would not consider a lymphoma to be causing your palpable lymph nodes.
You can monitor the size changes in your neck and seek medical advice if the lymph nodes start to increase in size to more than 2Cm.

Kidney bean: Normal cervical nodes are oval or flattened hypoechoic structures with varying amounts of hilar fat. The hilae are usually hypovascular. Malignant infiltration alters the features of the lymph nodes, resulting in enlarged nodes that are usually rounded and show peripheral or mixed vascularity. They may have thickened outer wall, internal echoes, cystic changes, internal nodules, and septations.

Yes: Most neck or cervical nodes can be visualized by ultrasound with characterization of texture, determination of size, and vasculature or hyperemia. Many times simple benign nodes are differentiated from infected and metastatic nodes.

Difficult if at all: Best way to tell is to take a biopsy and have it evaluated by a pathologist.

Normal exam: We all have lymph nodes in our necks. As long as they are not much larger than 15 mm then they are normal. "Reactive nodes" is the term to differentiate nodes that are just slightly larger because of an upper respiratory infection, for instance.

how come that i have an ongoing-chronic lymph nodes pain, while on ultrasound they are described as normal and not enlarged. infectious disease differential has been ruled out by testing twice in 6 mths.. i also have chronic fatigue and skin problems?

: My hunch is that they are not related. If you feel nodes a lot stop for a month and see if pain is gone. Chronic fatigue has many,, many causes. Maybe hundreds. Keep a diary to see if you can identify things that may be making it worse. Get blood K tested but has likely been done and OKl. Consider sleep apnea. Are you getting good sleep. Ask about a sleep study.

?Hard lymph nodes su: I would be suspicious if the lymph nodes are palpable and are hard when you feel them with your hand.
Ultrasound can be a reasonable screen for enlarged and even malignant lymph nodes, but it is not 100% reliable. So if the primary tumor is high risk for metastasis, i would want to surgically remove the lymph nodes for microscopic examination. That is the only 100% reliable test  for cancerdetecti.

Lower neck: Most likely this is referring to nodes located in the lower part of your neck closer to the sternum and clavicles.

Accepted Answer

A FNA: (fine needle aspiration) of the nodule and any node that looks suspicious on the ultrasound. See a thyroidologist now.
