Challenging: This can be challenging, but serial imaging could help. Scar tissue will remain stable while mass will be stable or increase in size depending on if it is malignant or benign

A few possibilities: Several things other than cancer can cause such abnormalities. Various infections and sarcoidosis come to mind. There's no way for a forum doctor to say anything more. The doctor(s) treating you or who did the bronchoscopy are the best to ask.

What kind of hernia?: It depends on the location of the hernia and what kind of surgery was done. Surgery for a hiatal hernia or diaphragmatic hernia may cause scar near the heart. But even this should not lead to a mass between the heart and lung.

Pulmonologist: I feel that you may have sarcoidosis and should be evaluated by a pulmonologist so that an accurate diagnosis will be made  through appropriate testing and proper treatment will be obtained.

Hard to say: A mobile soft tissue mass overlying the clavicle is most likely a lipoma, which is simply a benign mass of fatty tissue.  A lipoma can be easily excised if it's bothering you.  If you've had it for 2 years and it hasn't changed size or symptoms then it's probably not cancer.  Further workup such as mri, biopsy, or excision can be done to confirm whether or not it's cancer.

See your doctor: It could be either.  There are characteristic of masses that guide the radiologist one way or the other.  I would imagine that your doctor will recommend a biopsy and other tests to help decide what this is.

From infection: Caseous necrosis is form of cell death when  tissue maintains  cheese-like appearance. The dead tissue appears as a soft and white proteinaceous dead cell mass.Frequently encountered in tuberculous infections. Can  be caused by syphilis and certain fungi, histoplasmosis, cryptococcosis, and coccidioidomycosis.

No: Recurrent pneumonia can cause chronic scarring in the lung, but pneumonia itself not known to cause cancer or lung mass.
