Labs: Your thyroid test looks normal, but your parathyroid test looks high.  I would see an endocrinologist to go over those lab values.

Many causes: Indicates that ca is high and that PTH is not the cause.  Other causes include:
too much vitamin d
malignancy
chronic granulomatous disease
thiazides
hyperthyroid

rarely: acromegaly, addison's, milk alkali syndrome, and a few others.

High PTH: High PTH can be caused by a parathyroid gland tumor, a genetic condition called fhh (familial hypocalciuric hypercalcemia or low urinary calcium with high blood calcium), and vitamin d deficiency.  While fhh is usually asymptomatic,  high calcium from a parathyroid tumor, or vitamin d deficiency can cause many different symptoms.  An endocrinologist should be able to help find the cause.

Primary hyperparathy: If you have a high calcium and high PTH level, you most likely have primary hyperparathyroidism.This is a relatively common condition, especially in women after menopause. It has nothing to do with your diabetes.  In your age group, this would be treated by removing the abnormal parathyroid (likely 1 of 4 parathyroid glands is abnormal). Without treatment it can lead to kidney stones and bone loss.

No: If your calcium is normal and PTH is also normal, then there is no need to see anyone. The first value could be a lab error.  Check it again in the future. If calcium is still normal, then i wouldn't worry about this anymore. If it's high again, then consider working it up.

What symptoms?: What exactly are the symptoms? Calcium and ipth being normal there is no suggestion of hyperparathyroidism. Only other thing to check to make sure is phosphorus.

Borderline: Depends on your reference range of normal for PTH at th lab.  They sound slightly high to me, I would recommend seeing an endocrinologist to further tease out an answer.

Hypercalcemia: Hi. Non-parathyroid hypercalcemia (assuming normal kidney function) is usually due to granulomatous diseases or any number of malignancies; however, these conditions usually are NOT "mildly elevated", but tend to be more extreme, and usually with a frankly suppressed PTH. Serum calcium should be considered in context of albumin. Is your albumin on the high side? Is your ionized calcium high?

Several things: Low vitamin d levels, or kidney failure would be among the more common causes of low calcium and high PTH levels.

Accepted Answer

PTH should be low: If you calcium (and ionized calcium) is high, your PTH should be suppressed to low levels unless you have a parathyroid adenoma secreting excess pth. Supplementing low vitamin d is important. Also make sure you are not on thiazide diuretics. Your endocrinologist can evaluate the cause and treatment. Don't ignore it.
