Lymph node?: Could be a lymph node, which when enlarged, can indicate presence of an infection. If sexually active, suggest seeing your dr / urgent care / 'std clinic' for evaluation. If not, and otherwise feeling fine, can give it a few days--don't check too often, irritation can keep lymph node enlarged.

Try: a bit of steroid (Hydrocortisone) OTC (Over the Counter) cream...and wearing a condom can be a benefit in situations like this to avoid further trauma while the skin is healing

: He needs medical evaluation ASAP. The two most common causes of genital ulcers (sores) are herpes and syphilis. Do not have sex with him until he has been examined and tested for these as well s other STDs. And you need examination and testing yourself, since you might be the source of his problem. Good luck.

Possibly yeast: It's summer and we all sweat. He could have something as minor as a little yeast infection from the heat/sweating. You can try an over-the-counter antifungal cream. Have him air out area by not wearing underwear to bed at night for a week. Make sure if he's sweaty or hot to clean and dry area well. If bad rash or painful urination, needs to see doc for diagnosis or stronger treatment.

Syphilis? LGV?: Syphilis and lymphogranuloma venereum can cause a painless genital rash/sore. See your pcp to get checked ut, and also tested for other sexually transmitted infections with may coexist.
