Mutliple causes: Your symptoms sound like there is something going on in the blood vessels where tissues are not getting enough perfusion. Multiple sources can cause this including hx of smoking, diabetes, vascular diseases.  But turning black is not good sign and warrants urgent attention.  Have your doctor take a look and made need further specialty care.

Hard to say: As this most commonly would be related to vascular issues, possibly arterial or venous or both.  But his best bet is first see a doctor to better identify the cause, as it very well may not be related to anything previously mentioned, and to be able to better guide you/him, the actually cause of his symptoms are very important. Possible Blood Clot: I would advise that you go to the ER and have a ultrasound image for a blood clot.  If you have a clot in at artery, small portions of the clot can break off and occlude blood supply to the toe or toes.  You need to rule this out first and right away.

Depends: This truly depends on exactly what it looks like.   When cellulitis "burns out" with treatment, there is sometimes a brownish discoloration left behind.  However, brown/black always raises the concern of necrosis/gangrene, so if there are worsening symptoms such as pain or swelling, i would immediately go to the er.
