Fever : Fever , back pain. May be not leg pain. Talk to your doctor if symptoms persist.

Atrophy?: Atrophic vaginitis or desquamative vaginitis can cause the vaginal symptoms, as can other infections besides yeast, like bacteria and viruses. The treatments are all very different, so you'd need a gyn specialist to sort it out. As far as the llq pain, it may or may not be related, but it could be caused by a stone, or an ovarian cyst. But a kidney stone wouldn't cause vaginal burning.

Possible: The course is usually short, making it not a long lasting problem. However, antibiotics could have variable side effects that change from a person to another. For instance, cipro, (ciprofloxacin) a common antibiotic for UTI is known to cause musculoskeletal side effects as severe as rupture of muscle tendons, albeit rare.

Recurrent UTI: Not sure what "arm and leg" is about, unless maybe the cost of medical care for UTIs? There are easy steps to help prevent frequently recurring UTI in women. Ask doctor about possible antibiotic dose whenever you have sex, or perhaps on a regular basis e.g. 2-3 times a week. Perhaps avoid spermicides, diaphragm, and cervical cap if using them. Or an infectious disease specialist can help.

Urethritis or prosta: I suspect  you may have  inflammation of your urethra, or less likely prostate. Ask your physician to see a urologist.

Normally No.: A regular UTI cannot cause your legs to ache unless you are dehydrated. If symptoms continue to get worse it may be a sign of a more significant infection in your pelvic region. Please see your primary doctor if symptoms get worse or continue.

Sciatica ?: Since you have a history of kidney problem one always keep that in mind. However your pain is going down the leg which raises the possibility of sciatica due to irritation of nerves in the back. Discuss this possibility with your doctor

Musculoskeletal: Appendicitis is constant, worsens, causes fever, and declares itself in hours as it becomes a medical emergency.  The leg pain does not correlate with either appy or urine problem.  A pelvic girdle musculoskeletal disorder is implicated; with irritation of the sciatic nerve.  Another consideration is an hernia.  The best type doctors to diagnose either osteopath or physiatrist.
