Abnormal EKG: In a 21 year-old female should be looked at well. At your age, your risks for heart attack are low unless you have dm, chronic diseases like lupus, familial dyslipidemia , chronic HTN which is unlikely at your age, and smoking heavily. It has to be something congenital or infectious or normal variant. Talk to your doctor for diagnosis and treatment if needed and have peace of mind.

Get it checked out: If it is not movement related but does get worse with respiration then it is likely coming from your lung.  The causes can be pleuritis, pneumonia, a blood clot, etc, so best to have it looked at.

Shoulders complicatd: There can be many reasons for this - a muscle strain or joint sprain, a slightly torn tendon, and irritated bursa, an unstable shoulder joint, arthritis, torn cartilage, or shoulder parts jamming each other on motion.  This requires a careful shoulder examination by a rehab or orthopedic doc. Don't let this go on too long.

Do Some Research: This sounds like a "classic" case of a rotator cuff problem. That is, the tendons that move and/or stabilize the shoulder. If you're careful, it can heal on its own, but if there's been no progress after 4-6 months, go see an orthopedic doctor. Before that, avoid straining above shoulder height. Look on the internet for stretching and mobilizing exercises, use Motrin and apply ice after activity.

Exercise: Possibly a small cartilage or labral tear. Should get better over time. But I do recommend range of motion exercise like the pendulum exercise, taking Motrin or ibuprofen. See your doctor get checked if it does not get better over a few weeks.
