5 months duration ?: It is long need investigation , unknowingly you may have injured . some of the rotator  cuff injuries  patients do not know how they got injured   , see your doctor preferably orthopedic , for examination and tests possible MRI .

Arthritis: sometimes one nerve is more sensitive than the nerve on the opposite side.  The patients pain complaints trumps the radiological findings.

Shoulder pain: Possibilities include tendonitis of shoulder, torn rotator cuff and a bulging disk in the neck vertebra. Your symptoms appear positional. I would expect the shoulder pathology would also be noticeable during the day and with shoulder motions. Consult an orthopedic specialist. If no shoulder pathology is found, referral to a neurologist may be made.
