Many possibilities: In all honesty, an exam of his knee would yield many more detailed opinions. However, pain below the knee in a 13-year old is often Osgood-Schlatter syndrome. This occurs when the patellar tendon and growth plate attachment at the shin bone become inflamed. There is another similar condition at the lower base of the kneecap. A patella that is not tracking exactly right could give similar symptoms.

Cortisone Flare: Cortisone can sometimes cause a flare of joint pain 24-72 hours after injection. Typically this resolves and the benefits of the injection take effect after 72 hours. Joint infection is a small, but real risk of cortisone injection, especially in a diabetic patient. If your pain persists any longer, recommend re-evaluation by your orthopedic surgeon.

Runs w knee/leg pain: May be tendonitis, IT band ( from hip to knee), patella mal-alignment,patellofemoral syndrome, shin splits / stress fracture. Would check  for patella alignment,meniscus,  flat feet, check shoes that they are not worn, consider kinesiotape on knees, shins, ice, rest. Follow up with  ortho/ podiatry as you have to take a break from running if stress /fracture or shin splits

The treatment is to decrease your intensity/distance akin to a dimmer switch to just below the threshold where symptoms develop. If pain persists for much longer, seek an orthopaedic evaluation.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor. In the mean time you may take an over the counter pain medication like acetaminophen.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Sciatica: As you have described, my first diagnosis would be sciatic nerve irritation, or sciatica.  Many times this can be easily treated with physical therapy.  Go see your primary care physician for initial evaluation. That will determine if you need pt and even further evaluation with an mri.
