Disc Bulge: A disc herniation is a bulge of the disc. The position of the disc bulge/herniation and its impact on the spinal cord or nerve root that is important. Think of the disc as a tube of toothpaste with a hole, the tube as the annulus or outside of the disc and the paste as the nucleus, the inside of the disc. With a hole in the tube the paste begin to ooze out just like the disc nucleus.

Charite^: Although I have not had a replacement disc, I have performed this operation utilizing the charite^ artificial disc. When the patients are properly selected there is a very successful outcome. Additionally, for those cases where severe pain persist, a fusion remains a viable option. These "shocks" can wear out with routine life activities, age and injuries. They can also press on nerves.

No.: Disc herniation is the result of tearing of the ligamentous wrapping arounfd the disc, the annulus fibrosis. This can result from injury or be spontaneous. There is some evidence that degenerative disc disease can be hereditary. I am not aware of evidence of a heretitary component for disc herniation though. Someone may know otherwise.

Sciatica: Initially conservative treatment with Non-steroidal anti inflammatory pain medications, Epidural corticosteroid injections, Physical therapy, Spinal manipulation, Weight control, &amp; last resort is Surgical decompression &amp; repair, or Laser surgery.

Likely: Most "slipped" discs or disk herniations will resolve in time (90%). However if you develop weakness or sensory loss or loss of bladder control it is important to see a surgeon emergently.  Without those symptoms the best option is pt.
